Supporting people after discharge from the hospital and basic social counseling will be the main task of the new Return Line. According to Milena Johnová, Prague’s councilor for social policy and health care, the approach will be very individual. “The task of social workers will be to find out what the preferences and wishes of those callers are. Patients who are still in the hospital and expect to be released. And the wishes are usually such that they want to return home and do not know how to arrange it, “he adds.

The return line will provide patients and their families with advice on social benefits, a request for care, personal assistance or the transport of seniors and people with disabilities. It will provide information on field services, specialized workplaces and help with the selection of medical devices.

The social workers of the Return Line will systematically cooperate with the social departments of the city districts, branches of the Labor Office or social service providers. Above all, interest from older people is expected, but the service is intended for all ages. “Families with younger children may need social and health counseling. There are many non-profit organizations that can help them. So that’s why we have a structure there that we need to know what age it is. Each age category needs a slightly different help, with a different focus, ”said Iva Vyšatová, director of the Municipal Aftercare Hospital.
The hospital will evaluate the trial operation and in the future is considering expanding the services within the lines, such as rehabilitation.
