After the beginning of the school year, paediatricians’ surgeries are filled with children with respiratory infections – colds, coughs or sore throats. This was stated by doctors during a two-day congress of primary care in Prague. What do parents with children usually go to the surgery with? Are there more children in a certain age range?
We have really full surgeries. He goes especially with the symptoms of the common cold, which is a runny nose, cough, sore throat, fever, fever, fatigue. It’s hard to say if there are more children of any age group. But mostly preschool children and first grade children predominate.

You said that in the past, parents used to send children with colds or mild viruses to school. Now they’re going to the office with those cases. This is certainly due to covid, which initially has symptoms similar to virosis. So what do you recommend? How should parents proceed?
So we always recommended the same thing. There is nothing new that if the child is not well, does not feel well, has symptoms of infection, so leave the child at home in that restful home mode. And according to the symptoms, just give medication, whether it has a cold, cough, or to a temperature, if there is a high temperature, so really closely monitor the temperatures and clot them. And of course, if the condition does not improve or the parents do not know how to do it, then contact your doctor and have your child examined.
Can doctors recognize a child’s virus from covid at the beginning of the disease, or is a test always necessary?
It is very difficult to recognize. None of us will know because the symptoms are overlapping. What helps us a lot is the epidemic situation. That is, my mostly familiar family, and if my parents actually tell us what the health of the family members looks like, if there really was no contact or with acquaintances, with relatives visiting somewhere. If I know the situation in schools, we can combine these things and find out if the child needs to be tested or not. Also parents, if you weigh, you have shown a test of your child at home and in some way tries to rule out the fact that it would be a covid.

Je třeba rozdíl, pokud rodiče nechají otestovat dítě sami anebo na doporučení lékaře, chodí se do jiných laboratoří. Pokud třeba dítě odešle s podezřením na covid lékař, aby nebylo mezi těmi lidmi, kteří se nechávají testovat preventivně.
Pokud my indikujeme testování, tak ho provádíme ve svých ordinacích. A už ten samotný odběr měl být udělán tak, aby ten záchyt byl pokud možno spolehlivý. Další věc jsou spolehlivost testů. A samozřejmě pokud máme podezření na covid, pak odesíláme na PCR
Praktici mají nový doporučený postup. Měli by otestovat antigenním testem každého pacienta s příznaky, a tím případně prokázat covid, aby co nejdříve mohli zahájit léčbu. Jak budou postupovat dětští lékaři?
My máme trošičku odlišnou situaci v tom, že u nás většina těch pacientů, kteří přicházejí nemocní do ordinace, tak právě jsou s příznaky covidu. A jak ty počty narůstají, tak my samozřejmě vyšetříme každé dítě. Jen prosíme rodiče, aby se pokud možno domluvili na té návštěvě, aby nepřicházeli mezi novorozence nebo miminka nebo preventivní prohlídku, to je jedna věc. Druhá věc, určitě nejsme schopni otestovat každého s respiračními příznaky. Když si vezmeme, že třeba za dopoledne máme 25-30 nemocných dětí a jenom antigenní test trvá 10 až 15 minut. Tak to časově nelze zvládnout, proto budeme muset individuálně posoudit každého pacienta a rozhodnout, zda ten test je vhodný či ne.
Ale jak? Když jste sama říkala, že na počátku to vlastně není poznat, o co jde. Není vlastně test důležitý pro to, aby dítě nešířilo případný covid dále a neměli jsme tu třeba znovu zavřené třídy, školy?
Pokud to dítě zůstane doma a v péči rodiče, počká třeba jeden, dva dny. I ten test nemusí být první hodiny příznaku dělaný, tak to je jedna věc. Druhá věc je, že pokud vím, že to dítě je třeba školkové, pohybuje se jen ve školce a v rodině. Rodiče mi řeknou, že nikde jinde nebyly a že nemá mimo školu podle aktivity. A vím, že ve školce není žádný případ covidu. Nebo že v rodině prodělali covid. Tak ho nebudu prioritně testovat a počkám, jak se bude jeho stav vyvíjet.
Takže je skutečně důležité zůstat doma a neposílat děti, kterým není úplně dobře, do školy a školky.
To určitě. A potom, pokud se stav nelepší, tak se určitě obrátit na svého lékaře a nechat se vyšetřit.
